Our streamlined onboarding process is designed to make it quick and easy for you to access the support you need. We’ve simplified the steps to ensure you get connected to your vital services without delay, allowing you to focus on achieving your goals and thriving in your community.
✅ you or your representative will book a online or face to face free consultation with a Support Relationship Manager or an iSeekCare Franchise Manager.
✅ Complete a client onboarding form from our website.
✅ We will collect necessary documents (NDIS plan, medical reports, etc.)
✅ We will prepare a detailed assessment with you on your support needs, goals, and preferences
✅ We will prepare your service agreement for you to review and make any necessary adjustments.
✅ Review and sign your service agreement
✅ Prepare your support plan, support procedures and any complex care plans in lines with your goals and requirements.
✅ Complete all online forms such consent form, etc
✅ Setup your online portal access.
✅ We will prepare to implement your support (e.g. arranging approved workers, scheduling appointments)
✅ All support workers are trained and briefed on your support plans.
✅ We will introduce your support workers and key provider contacts.
✅ Your support manager will attend the first shift to ensure things are done right.
✅ Make necessary adjustments to the support plan as needed by you.
✅ We will schedule regular check-ins (monthly or quarterly) to assess satisfaction and progress.
✅ Continue to adjust supports as the participant’s needs evolve.
✅ Prepare for the annual NDIS plan review, gathering feedback and documentation to support the review process.
✅ Your support manager or iSeekCare franchise will invite you to provider events and continue to be available for you whenever you need.
